Preparing for Your Appointment

To ensure a smooth and efficient notarization, please review the checklist below before your appointment time.

Bring a Valid, Unexpired Government‑Issued ID

Acceptable forms include:

  • Driver’s license

  • State ID

  • Passport

  • Military ID

The name on the ID must match the name on the document.

Have Your Document Fully Completed (Except Signatures)

Please ensure:

  • All blanks are filled in

  • All pages are present

  • No corrections are needed

  • You have read and understand the document

Notaries cannot provide legal advice or tell you what to write.

All Signers Must Be Present

Every person signing the document must:

  • Be present

  • Have valid ID

  • Be alert, willing, and aware

If a signer is not ready at the appointment time, wait‑time fees apply.

Witnesses (if required)

  • MLC Notary Services does NOT provide witnesses for in‑person appointments

  • Clients must bring their own witnesses

  • Witnesses must be:

    -18 or older

    -Not named in the document

    -Not benefiting from the document

For RON appointments, a witness may be available on a limited basis and must be confirmed in advance.

For Facility Appointments (Hospital, ALF, Nursing Home)

Please ensure:

  • Staff is aware of the appointment

  • The signer is awake, coherent, and able to sign

  • The signer has their ID available

  • Any required staff forms are completed beforehand

If the signer is unable to sign or lacks ID, or not coherent the appointment cannot proceed.
